Care Packages
At Tiny Hero, we know how overwhelming it can be to hear your child’s CDH diagnosis, but we are here to support you along the way. We want to welcome you into our family and offer our support with a Tiny Hero CDH Care Package.
We have put together a fun package of items for newly diagnosed families to help brighten the way and assist you on your CDH journey. We can’t wait to share one with you! Please note, we do not send care packages outside of the USA or to those 2 months post delivery.
Guidelines
Each month, Tiny Hero sends out as many as 15 care packages to the families of newly diagnosed or delivered CDH babies. Before requesting a care package, please review the following guidelines to ensure that you meet the criteria.
You may apply for a care package upon receiving a diagnosis of CDH and up to two months after your CDH baby’s delivery.
All requests must be submitted via our Care Package Form, found below. No emails or physical letters will be accepted.
Only ONE care package per household. Any resubmitted or duplicated requests will be removed from our waitlist. Households located outside the United States are unable to be considered at this time.
If more than 15 care packages have been requested for a given month, the packages will delivered in order of submission, so make sure to request yours today!
